The WP:CRITERIA state that article titles should be 'recognisable' and 'natural'. No native English speaker is going to know what Súdwest-Fryslân is, but 'Southwest Friesland' is instantly recognisable, hence we have Friesland and not Fryslân. Just because the name may be complicated to non-West Frisian speakers, doesn't mean it should be translated into English though. To me, the most natural title would be its official name – unless a different name is more commonly used in English-language sources (cf. The Hague).
