Had a look through the last 3 years' worth of Warship International in the hopes of finding any mentions of the ship's activity in the Naval Intelligence section, but no such luck. There may well be details in earlier issues, but I didn't have those immediately handy.

"pendant number" or "pennant number"? Not sure I've heard the former term
Amended and linked. The former is used by Warlow & Bush in their title Pendant Numbers of the Royal Navy.
The career section is a bit out of order - the 1995 refit is listed before the 1986 exercises, then we have events in 2011, and then back to 1990.
